Keep the idea.
Find the first move.
A rough thought does not need a perfect plan. Save enough context to understand it later, then choose the smallest useful way to explore it.
Save what made you notice it.
“Fix onboarding” might make sense now and mean very little next week. Record the moment behind the idea: who was stuck, what they expected, and what you saw happen.
Separate what you observed from what you inferred. “Two people asked who owned the review” is an observation. “Nobody understands the process” is a conclusion that needs checking.
A small note with enough context.
Use these prompts in any notes app. A few sentences are enough; leave a field open when you do not know.

Keep the note tentative. Capturing a possible change does not commit you or anyone else to doing it.
Find the question underneath the idea.
Suppose the same handoff causes repeated questions. Before designing a new process, find out whether ownership is unclear, the information is missing, or the current instructions are hard to find.
Each explanation suggests a different next step. Bring the original note and relevant conversation back together before choosing one.
Choose a step that teaches you something.
Ask the current owner how the handoff works. Find one recent example. Try a clearer instruction with the next person who needs it. Choose an action small enough to do, with a question it should answer.
If someone accepts a deliverable or date, record that as a commitment. Keep it separate from the ideas you are still exploring. Review the note when you learn something; it is fine to decide the idea is not worth pursuing.
